Corporations will require the following information:

1. Employer Identification Number (EIN) issued by the IRS in the name of the business
2. Filed Articles of Incorporation with the State of CA
3. Current Fictitious Business Name Statement (FBNS), if applicable
   NOTE: FBNS is required if the business name is different than on the Articles (operating under a DBA of the Articles)
4. Online verification of entity from CA Secretary of State (CA SOS)
5. Resolution of Authority
6. Certification of Beneficial Owner(s)

General Partnerships will Require the following information:

1. Employer Identification Number (EIN) issued by the IRS in the name of the business
2. General Partnership Agreement signed by all Partners (required if the business is not filed with the county or state)
3. Current Fictitious Business Name Statement OR Statement of Partnership Authority (GP-1) filed with the State of CA. NOTE: FBNS is not required when each of the owners' surname/last names are used in the business name. However, if the business name implies the existence of other business owners, a FBNS is required.
4. Online verification of entity from county or state, if applicable and available
5. Resolution of Authority
6. Certification of Beneficial Owner(s)

Limited Liability Companies will require the following information:

1. Employer Identification Number (EIN) issued by the IRS in the name of the business
   NOTE: Single member LLC can use TIN
2. Current filed Articles of Organization (LLC-1) with the State of CA
3. Operating Agreement
4. Current Fictitious Business Name Statement (FBNS), if applicable
   NOTE: FBNS is required if the business name is different than on the LLC-1 (operating under a DBA of the LLC)
5. Online verification of entity from CA Secretary of State (CA SOS)

7. Certification of Beneficial Owner(s)

Limited Liability Partnerships will require the following information:

1. Employer Identification Number (EIN) issued by the IRS in the name of the business
2. Filed Application to Register a Limited Liability Partnership (LLP-1) with the State of CA
3. Limited Liability Partnership Agreement signed by all Partners
4. Operating agreement
5. Current Fictitious Business Name Statement (FBNS), if applicable
   NOTE: FBNS is required if the business name is different than on the LLP-1 (operating under a DBA of the LLP)

Sole Proprietorships will require the following information:

1. Tax Identification Number (TIN)
2. Social Security Number (SSN) of the owner of the business OR
3. Individual Taxpayer Identification Number (ITIN) of the owner of the business OR
4. Employer Identification Number (EIN) issued by the IRS in the name of the business or in the name of the owner, if applicable
5. Business or occupational license, if applicable
6. Current Fictitious Business Name Statement (FBNS) filed within the last 5 years in the county the business is located (if applicable)
   NOTE: FBNS is not required when the owner's surname/last name is used in the business name, such as Bates Pool Care or Thompson's Handyman Service. However, if the business name implies other business owners, i.e. Smith & Sons, Anderson & Company, or Jones and Associates, etc., then a FBNS is required.
7. Online verification of entity from city or county, if available

Unincorporated Non-Profit Associations or Unincorporated Associations will require the following information:
NOTE: If the Non-Profit Association is incorporated, refer to the Corporation section.

1. Employer Identification Number (EIN) issued by the IRS in the name of the organization/association
2. Letter or meeting minutes authorizing account opening
3. Governing documents (Charter, Bylaws, and/or Meeting Minutes) stating elected officers, signed, and dated by the Secretary
4. IRS Letter confirming 501(c)(3) status, if applicable
5. Resolution of Authority
6. Certification of Beneficial Owner(s)
